All CMP offices have access to a comprehensive suite of services offered through our Centralized Programs department. These services are designed to support high-quality, coordinated care and include the following core functions:
Chronic Care Management (CCM)
Patients enrolled in our Chronic Care Management program receive monthly outreach calls from our dedicated CCM team. We provide comprehensive support to help you achieve your health goals, including guidance on nutrition, physical activity, medication adherence, and addressing gaps in your care such as overdue screenings or missed appointments.
Transitions of Care (TOC)
Our Transitions of Care team works closely with your primary care provider’s office to help coordinate follow-up care after an urgent care visit, emergency room visit, or hospital stay.
